Limited disclosure and confidentiality agreements are standard practices. Information is disclosed to employees, consultants and contractors only on a need-to-know basis, and when necessary, all parties must sign a confidentiality agreement under which they must maintain the confidentiality of the designated information.
Storage of all confidential information in paper, CD, tape and video format is maintained in locked rooms with access restricted to only senior project managers. Confidential information is not permitted to be removed from Health Outcomes Group premises. Access to confidential information that is stored in electronic form on computer systems requires the use of a password or special code that is given only to certain persons to access.
Medical records are stored and maintained in accordance with the Guidelines established by the American Statistical Association (ASA). Any dissemination of data at the level of an individual patient or in summary form respects the privacy and confidentiality of patients and also conforms to the Guidelines of the ASA, AMA and individual IRB who has approved the study from which the data originated.
Restricted access is maintained by prohibiting outsiders and visitors from entering areas where confidential information is maintained.
